Hospice Registered Nurse Case Manager (RN)
About the role
As a Hospice Registered Nurse Case Manager (RN), you will assist the team with on-call, case management, and visits to ensure adequate coverage and call-outs. Your duties will include completing initial assessments of patients and families to determine hospice care needs, regularly re-evaluating patients' nursing needs, using assessment data to determine care plan needs, collaborating with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care, counseling patients and families in meeting end-of-life needs, initiating appropriate preventative and palliative nursing procedures, communicating with physicians regarding patients' needs and reporting any changes in patients' conditions, and supervising Hospice Aides.
Requirements
- Associate's Degree in Nursing
- Must have a State Certified RN license
- 2 years' experience as a professional nurse, hospice experience preferred
- Must have a valid Driver's license and reliable transportation
- Willingness to travel
- Strong verbal and written communication skills
