Hospice Registered Nurse Case Manager (RN)
About the role
As a Hospice Registered Nurse Case Manager (RN), you will assist the Team with On Call/Case Management/Visits to aid in coverage and call outs. Your duties will include completing an initial assessment of patient and family to determine hospice care needs, regularly re-evaluating the patients nursing needs, using assessment data to determine care plan needs, collaborating with the entire healthcare team, patient and family to help direct the work of the team to provide care that is continuous and well-coordinated, counseling the patient and family in meeting end of life needs, initiating appropriate preventative and palliative nursing procedures, communicating with the physician regarding patient's needs and reporting any changes in the patient's condition, and supervising the Hospice Aides.
Requirements
- Associate's Degree in Nursing
- Must have a State Certified RN license
- 2 years' experience as a professional nurse, hospice experience preferred
- Must have a valid Driver's license and reliable transportation
- Willingness to travel
- Strong verbal and written communication skills
