About the role
As a Hospice Registered Nurse Case Manager, you will assist the team with on-call duties, case management, and patient visits to ensure coverage and address call-outs. This position offers a flexible and personalized schedule with a strong emphasis on work-life balance, operating Monday through Friday with no overnights or weekends.
Responsibilities
- Complete an initial assessment of patient and family to determine hospice care needs.
- Regularly re-evaluate the patients' nursing needs.
- Use assessment data to determine care plan needs.
- Collaborate with the entire healthcare team, patient, and family to direct the team’s work and provide continuous, well-coordinated care.
- Counsel the patient and family in meeting end-of-life needs.
- Initiate appropriate preventative and palliative nursing procedures.
- Communicate with the physician regarding patient needs and report any changes in the patient’s condition.
- Supervise Hospice Aides.
Requirements
- Associate's Degree in Nursing.
- State Certified RN license.
- 2 years of professional nursing experience; hospice experience preferred.
- Valid Driver’s license and reliable transportation.
- Willingness to travel.
- Strong verbal and written communication skills.
