About the role
The RN Case Manager will assist the team with on-call coverage, case management, and patient visits to ensure continuity of care.
Responsibilities
- Complete an initial assessment of patient and family to determine hospice care needs
- Regularly re-evaluate the patient’s nursing needs
- Use assessment data to determine care plan needs
- Collaborate with the entire healthcare team, patient, and family to direct the team’s work and provide continuous, well-coordinated care
- Counsel the patient and family in meeting end-of-life needs
- Initiate appropriate preventative and palliative nursing procedures
- Communicate with the physician regarding patient’s needs and report any changes in the patient’s condition
- Supervise Hospice Aides
Requirements
- Associate’s Degree in Nursing
- State Certified RN license
- 2 years’ experience as a professional nurse (hospice experience preferred)
- Valid Driver’s license and reliable transportation
- Willingness to travel
- Strong verbal and written communication skills
