About the role
The Patient/Staff Service Coordinator position plays a crucial role in ensuring efficient and effective patient care coordination within home health, home care, and hospice services. This role involves managing schedules, communicating with staff and patients, and maintaining compliance with regulatory standards.
Responsibilities
- Uses the EMR system to prepare and assign staff schedules for home health, home care, and hospice visits, including therapy services.
- Routes and assigns field staff to minimize drive time and mileage, and maximize productivity.
- Adjusts schedules based on staff availability and patients' changing needs.
- Processes all schedule requests, including frequency changes and add-on disciplines.
- Schedules the entire certification period at the start of the certification period.
- Enters patients' descriptions in the EMR system and completes the requested schedule for admissions.
- Schedules continuation of services for recertifications.
- Reviews schedule notifications and designates correct billing codes for each visit.
- Maintains communication with patients, caregivers, and discharge planners regarding patients' status when hospitalized.
- Participates in case conferences and assists in obtaining reauthorizations from insurance companies.
- Schedules on-call visits, receives calls from the answering service, verifies therapy visits, uploads paper visits to scheduled visits, and arranges staffing to cover call-offs.
Qualifications
- High school diploma or equivalent required.
- Post-secondary education in a related field preferred.
- Two years of experience in staff scheduling.
- One year of experience in home health, hospice, or medical office management preferred.
- Demonstrated ability to manage multiple priorities simultaneously.
- Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
- Proficiency with electronic medical records (EMR) system preferred.
Requirements
- Must be able to read, write, speak, and understand the English language.
- Subject to emotionally upset and hostile patients, family members, etc. - minimal risk.
- Risk category for exposure to bloodborne diseases - III.
