Job Description
The Hospice Scheduler works with the clinical team to ensure patients are scheduled based on need and physician ordered frequency. Delivers administrative support functions as needed to the Hospice clinical team. Reviews staff schedules on a daily basis and notifies the clinical supervisor of scheduling volume concerns and changes that need to be made.
- Provides administrative support and assistance to the clinical team as needed including answering incoming calls, taking accurate messages, follow up communication, and running reports as needed.
- Ensures SNVs are scheduled per frequency order.
- Provides scheduling support to all interdisciplinary team members including RN, Social worker, and Chaplain.
- Schedules Hospice Aide patients based on geography and patient need, ensuring orders and hospice aide care plan are in place.
- Tracks and reviews field staff daily schedules, notifying the Manager of under/over scheduling for individuals.
- Aids in weekend work/scheduling communications.
- Communicates schedule of Hospice Aide to Long Term Care Facility.
- Aids in holiday visit scheduling preparation.
- Documents telephone interactions in the EMR. Completes documentation in accordance with agency timeliness policies.
- Communicates with and provides back-up coverage and support to other associates in the department, this includes coverage of the agency holiday.
- Maintains mileage sheets for Hospice Aides.
- Maintains strict adherence to the Continuing Care Confidentiality policy set by the agency and federal HIPAA guidelines.
- Complies with all BID Plymouth/BILH policies and procedures.

- Identifies specific learning needs and goals and collaborates with Clinical Supervisor in developing a plan to meet them.
- Participates in departmental and/or interdepartmental quality improvement activities.
- Performs all other duties as needed or directed to meet the needs of the department.
Education/Experience Required
- High School Graduate or GED
- Experience in the medical field and/or office administration
- At least one year of scheduling experience preferably in a Home Health or Hospice Setting
