About the role
This position involves providing compassionate care as a Hospice Homecare Case Manager Registered Nurse, focusing on patient-centered support and clinical management.
Responsibilities
- Assessing and coordinating patient care plans to ensure comfort and quality of life.
- Collaborating with healthcare teams to deliver personalized services in a homecare setting.
- Monitoring patient progress and adjusting care strategies accordingly.
- Educating patients and families about care options and symptom management.
- Maintaining accurate documentation and complying with healthcare regulations.
Requirements
- Registered Nurse (RN) licensure required.
- Basic Cardiac Life Support (BCLS) certification within 30 days of hire.
- At least 1 year of relevant healthcare experience.
- Valid Driver License obtained within 30 days of employment.
