Role Overview
The Hospice Homecare Case Manager Registered Nurse provides compassionate, patient-centered care in a home setting, coordinating services to support patients and their families through end-of-life care.
Responsibilities
- Assess patient needs and develop individualized care plans
- Coordinate with multidisciplinary teams to ensure comprehensive care delivery
- Educate patients and families about disease management and care options
- Monitor patient progress and adjust care plans accordingly
- Document care activities accurately and maintain compliance with healthcare regulations
Qualifications & Skills
- Registered Nurse (RN) license required
- Associate Degree in Nursing or higher
- Minimum of 1 year of relevant experience
- Basic Cardiac Life Support (BCLS) certification within 30 days of hire
- Valid Driver License within 30 days of employment
- Strong communication, organizational, and compassionate skills
