Responsibilities

• Deliver high-quality post-acute care, including rehabilitation, home health, hospice, and palliative services.
• Utilize advanced technologies, such as SPOT Technology, for early detection and response, particularly in stroke and sepsis cases.
• Collaborate with multidisciplinary teams to improve clinical outcomes and patient recovery.
• Support patients and families with warmth, dignity, and personalized care.

Requirements

• Registered Nurse (RN) licensure, with Basic Cardiac Life Support certification within 30 days of employment.
• Associate Degree with at least 1 year of relevant experience.
• Valid Driver License within 30 days of start date.
