About the Role
We are seeking a Registered Nurse for a Hospice Homecare Case Manager position. This role focuses on transforming care delivery and improving clinical outcomes in a collaborative environment, while providing care with warmth, compassion, and dignity.
Responsibilities
- Provide compassionate post-acute care, including hospice and home health services.
- Collaborate with healthcare teams to coordinate patient care plans.
- Ensure compliance with clinical protocols and regulatory standards.
- Support patients and families through education and emotional support.
Requirements
- Associate Degree in Nursing or higher.
- Valid RN license and Driver License (obtain within 30 days if not already held).
- Basic Cardiac Life Support certification (must be obtained within 30 days).
- Minimum of 1 year of relevant experience.
