Hospice Education Specialist

Responsibilities

• Build and maintain relationships with healthcare referral sources such as physicians, hospitals, and senior living communities.
• Conduct visits and calls to promote awareness of hospice services and facilitate patient referrals.
• Represent the organization at community events, health fairs, and networking opportunities.
• Deliver educational presentations to healthcare professionals and community leaders about hospice care benefits.
• Communicate eligibility criteria, care philosophy, and advocate for patient and family access to services.
• Collaborate with clinical teams to ensure smooth referral-to-admission processes.
• Support service growth by providing market feedback to leadership.
• Meet or exceed referral and admission goals, document activities in CRM per policies, and participate in ongoing education.
• Perform related tasks, adhere to policies, maintain confidentiality, and report incidents as required.

Requirements
