Hospice CNA Per Diem

What You'll Do:

  • Provide hands-on assistance with daily living activities such as bathing, grooming, dressing, toileting, and mobility.
  • Monitor and report changes in patient condition to the hospice nurse or care team, ensuring timely interventions.
  • Deliver emotional, spiritual, and compassionate support to patients and their families, fostering a sense of comfort and understanding.
  • Help families navigate the hospice process, offering guidance and reassurance during difficult decisions and transitions.
  • Maintain a safe, clean, and organized environment for patients, promoting comfort and dignity at all times.
  • Document care provided accurately and timely and communicate effectively with the interdisciplinary team.

What We're Looking For:

  • Compassion, empathy, and patience in caring for patients and their families.
  • Strong communication skills to collaborate with patients, families, and the care team.
  • Reliability and flexibility to meet the needs of patients in a dynamic care environment.
  • Experience as a CNA, HHA, or similar role is preferred, but a heartfelt dedication to caring for others is essential.
  • Able to provide comfort, respect, and advocacy for patients in end-of-life or home health care.

Location and Hours:

  • The job requires driving to where the patient lives which could be a nursing home, an assisted living facility, or a private residence.
  • The main home office is in Marlborough, MA.
  • Must have a reliable vehicle.
  • Per Diem/as needed.
  • There is some flexibility on the hours.
  • Rotating holiday and limited weekend schedule.

Experience:

  • Minimum of one year of experience as a Certified Nursing Assistant.
  • Hospice experience preferred.

Skills:

  • Must be able to read and write in English and follow written and verbal instructions in English.
  • Demonstrates interest in the welfare of ill and elderly.
  • Must pass Aide Competency Evaluation Skills.

Education:

  • High School Diploma preferred.
