About the role
This part time (32 hours/week) position will be working on Saturday and Sunday with flexible weekdays. This role will be traveling to see hospice patients primarily in East and West Portland areas.
Responsibilities
- Complete medical requirements while coordinating a whole-person plan of care according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act.
- Visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT).
Requirements
- Education sufficient for licensure
- Upon hire: Oregon Registered Nurse License
- Within 60 days of hire: Washington Registered Nurse License
- Upon hire: National Provider BLS - American Heart Association
- Upon hire: In compliance with the Providence Health and Services-OR, criminal background check is required for this position, based on assigned duties
- 2 years Acute care and/or hospice, home health, or long-term care RN experience.
