About the Role

Journey Hospice has a rewarding and fulfilling position for a full-time RN Case Manager. The Hospice RN will oversee a primary caseload of Hospice patients and provide skilled professional nursing care to them, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures.

You will provide the initial assessment and comprehensive nursing assessment, identify patient problems, and collaborate to develop the Hospice Interdisciplinary Team (IDT) plan of care. You will implement and reevaluate the plan of care relative to the patient’s nursing needs, ensuring appropriate, comprehensive, and responsive goals and interventions. Additionally, you will teach, supervise, and counsel patients, family members, and staff regarding nursing care and related patient needs at home.

Working closely with other members of the IDT, you will ensure all patient and family care needs are met and supervise Hospice Aides/Homemakers.

Responsibilities

• Perform initial and comprehensive nursing assessments and identify patient problems.
• Collaborate with the Hospice Interdisciplinary Team (IDT) to develop and revise the plan of care.
• Implement and reevaluate the plan of care to meet patient nursing needs.
• Teach, supervise, and counsel patients, families, and staff regarding nursing care and patient needs.
• Work closely with IDT members to ensure all patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
• Demonstrate understanding of Hospice philosophy and principles of death/dying.

Requirements

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid driver's license and reliable transportation insured in accordance with NJ state requirements.

Qualifications

• Minimum of one (1) year of general nursing experience preferred, with current hospice, home health, medical, surgical, or critical care experience.
• Experience with pain and symptom management interventions preferred.
• Ability to work within an interdisciplinary setting.

Pay

This position offers a competitive salary between $90,000 and $95,000.
