About the Role

Full‑time Registered Nurse (RN) Case Manager serving the Lafayette, IN area for Anew Hospice, a family‑owned company operating exclusively in Indiana. The RN delivers patient care through the nursing process of assessment, planning, implementation, and evaluation, and coordinates hospice services in patients’ homes and communities.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process.
• Plan, organize, and direct hospice care services.
• Provide community health education and leverage community resources to meet the needs of individuals and families.
• Deliver compassionate service with integrity and respect.

Requirements

• Valid Indiana Registered Nurse (RN) license.
• Previous hospice or long‑term care experience preferred.
