Hospice RN Case Manager
Journey Hospice is seeking a per diem RN Case Manager to work in Atlantic County. The position offers an hourly rate of $45.00 and flexible benefits.
Responsibilities
- Oversight of a primary caseload of Hospice patients
- Provides skilled professional nursing care to patients, families, and caregivers as directed by the physician
- Develops and implements the Hospice Interdisciplinary Team (IDT) plan of care
- Assesses and reassesses nursing needs and collaborates with the IDT to revise the plan of care
- Teaches, supervises, and counsels patients, family members, and staff on nursing care and patient needs
- Works closely with other members of the IDT to ensure all patient and family care needs are met
Requirements
- Graduate of an accredited registered nursing program
- Bachelor’s degree in Nursing preferred
- Certified Hospice and Palliative Nurse (CHPN) certification preferred
- NJ RN Licensure by the NJ State Board of Nursing in good standing
- Valid drivers license and reliable transportation that is insured in accordance with NJ state requirements

Experience
- One (1) year general nursing experience preferred
- Current hospice, home health, medical, surgical, or critical care experience
- Experience with pain and symptom management interventions preferred
- Ability to work within an interdisciplinary setting
- Understanding of Hospice philosophy and principles of death/dying
