About the Role
Journey Hospice has a rewarding and fulfilling position for a Full-Time RN Case Manager to serve our clients in Adams and Franklin Counties. The Hospice RN will oversee a primary caseload of Hospice patients and provide skilled professional nursing care to them, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, PA Hospice licensure laws, accrediting body standards, and agency policies and procedures.

Responsibilities
- Provide the initial assessment and comprehensive nursing assessment and identification of problems for each patient.
- Collaborate to develop the Hospice Interdisciplinary Team (IDT) plan of care.
- Implement the plan of care and reevaluate and revise it relative to the patient’s nursing needs to provide appropriate, comprehensive, and responsive goals and interventions.
- Teach, supervise, and counsel the patient, family members, and staff regarding nursing care and the patient's needs, including other related problems at home.
- Work closely with other members of the IDT to ensure all patient and family care needs are being met.
- Provide supervision to Hospice Aides/Homemakers.
Requirements
- Graduate of an accredited registered nursing program.
- Bachelor’s degree in Nursing preferred.
- PA RN Licensure by the PA State Board of Nursing in good standing.
- Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.

- Experience with pain and symptom management interventions preferred.
- Ability to work within an interdisciplinary setting.
- Understanding of Hospice philosophy and principles of death/dying.
Qualifications
- Minimum of one (1) year of general nursing experience preferred with current hospice, home health, medical, surgical, or critical care experience.
