About the role

As a Per Diem Staff Nurse Hospice serving Monmouth County, you’ll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.
Responsibilities
- Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
- Assess patients with advanced illnesses and create individualized hospice care plans.
- Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
- Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
- Document patient status updates and adjust care plans as needs and goals evolve.
Qualifications
- RN with Bachelor’s degree in Nursing
- Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
- 1+ year of supervised clinical experience as a registered nurse
- Excellent communication skills
- Strong computer skills
- Excellent organizational and interpersonal skills
Working Conditions/Physical Demand
Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
Pay
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $51.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
