About the role
The Hospice Access Field and Triage RN is responsible for conducting patient assessments, developing individualized care plans, and coordinating hospice services. In the field, they visit patients at home or facilities to evaluate eligibility, present hospice programs, and collaborate with the interdisciplinary team, family, and caregivers. In triage, they handle patient calls from the NE Portland location, providing urgent and routine support.
Responsibilities
- Assess patient needs and coordinate comprehensive, person-centered care
- Conduct home visits for eligibility and care presentations
- Collaborate with healthcare team and caregivers
- Manage patient calls for triage and support
- Follow nursing practices aligned with licensure and regulations
Qualifications & Skills
- Oregon RN licensure and within 6 months, Washington RN license
- National Provider BLS certification
- Minimum 2 years of acute care, hospice, home health, or long-term care RN experience
- Preferred: Bachelor's Degree in Nursing and call center experience
