WesleyLife at Home is seeking an RN with experience in home health, wound care, skilled nursing, infusion therapy, or catheter care. Our RNs help older adults remain independent, confident, and well in the place they feel safest—their home. WesleyLife Home Health provides comprehensive home health care services throughout central Iowa, serving clients in Boone, Dallas, Hamilton, Jasper, Marion, Marshall, Polk, Story, Warren, Cass, Washington, Audubon, Guthrie, Adair, and Poweshiek counties. Our services include medical support, therapy, and assistance with daily activities, delivered in clients' homes to enhance quality of life, promote independence, and support well-being.
Responsibilities
- Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in-home visits.
- Evaluate client progress, response to treatment, and ongoing needs to ensure high-quality, person-centered care.
- Counsel clients and their families on health conditions, care plans, medication management, and other related needs.
- Provide clear, compassionate guidance that empowers families and supports safety and independence at home.
- Maintain ongoing communication with the Nurse Case Manager regarding updates to healthcare provider orders, changes in client condition, incident reports, etc.
- Collaborate with the interdisciplinary team to ensure seamless, coordinated care.
