Home Health RN (Registered Nurse) - Irwin, PA - Weekdays & Full-Time

About the Role

Home health nursing done right means you own your patients, not a punch-list of drive-by visits. You do the full assessment, you build the plan, you see it through—with a team behind you and a company that treats your time as worth something. This full-time, weekday position is based in Irwin, PA, and involves traveling to patients in their homes or facilities.

Responsibilities

• Provide admission, care management, and follow-up visits utilizing the nursing process: assess, diagnose, plan, implement, evaluate.
• Utilize our brand-new Artificial Intelligence system to minimize documentation and keep patient care the focus.
• Build and maintain the plan of care for new patients in collaboration with our interdisciplinary team.
• Deliver hands-on nursing care, including physical assessment, medication management and administration, wound care, IV therapy, tube feeding, durable medical equipment setup and training, patient education, and comfort care.
• Coordinate care with physicians, PAs, nurse practitioners, home health aides, physical and occupational therapists, and other clinicians.
• Keep each patient's physician updated on care needs and progress.
• Supervise LPNs, home health aides, and personal home care aides as needed.
• Teach patients and families home care procedures and safety strategies.
• Maintain full compliance with HIPAA and all state and federal regulations.
• Document clinical notes clearly and concisely.

Requirements

• Active RN license in the State of Pennsylvania.
• CPR certification.
• At least 1 year of experience as a registered nurse.
• Strong organizational and communication skills.
• Reliable transportation, a valid driver's license, and insurance.
• Availability for a minimal on-call rotation, including weekends, evenings, and holidays.

Prior home health experience preferred.
