About the role

Anova Home Healthcare Services is seeking a compassionate and driven Registered Nurse to provide high-quality care to patients in their homes. The role involves providing comprehensive care, utilizing AI systems, coordinating with interdisciplinary teams, and maintaining compliance with regulations.

Responsibilities

  • Provide admission, care management, and follow-up visits using the nursing process.
  • Assess, diagnose, plan, implement, and evaluate patient care plans.
  • Utilize AI system for minimal documentation and focus on patient care.
  • Build and maintain the plan of care for new patients with the interdisciplinary team.
  • Deliver hands-on nursing care, including physical assessments, medication management, wound care, IV therapy, tube feeding, durable medical equipment setup and training, patient education, and comfort care.
  • Coordinate care with physicians, PAs, nurse practitioners, home health aides, physical and occupational therapists, and other clinicians.
  • Travel to patients' homes or facilities to provide care.
  • Keep patients' physicians updated on care needs and progress.
  • Supervise LPNs, home health aides, and personal home care aides as needed.
  • Teach patients and families home care procedures and safety strategies.
  • Maintain full compliance with HIPAA and all state and federal regulations.
  • Document clinical notes clearly and concisely.

Requirements

  • Active RN license in the State of Pennsylvania.
  • CPR certification.
  • At least 1 year of experience as a registered nurse.
  • Strong organizational and communication skills.
  • Reliable transportation, valid driver's license, and insurance.
  • Availability for a minimal on-call rotation, including weekends, evenings, and holidays.
  • Prior home health experience preferred.

Qualifications

  • High school diploma or equivalent.
  • Bachelor's degree in Nursing preferred.

Skills

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in Microsoft Office.
  • Knowledge of home health care standards and regulations.
