What You'll Do Provide admission, care management, and follow-up visits utilizing your nursing process: assess, diagnose, plan, implement, evaluate.Utilize our brand-new Artificial Intelligence system to help keep documentation minimal and patient care your focus.Build the plan of care for new patients and keep it current with our interdisciplinary team.Deliver hands-on nursing care, including physical assessment, medication management and administration, wound care, IV therapy, tube feeding, durable medical equipment setup and training, patient education, and comfort care.Coordinate care with physicians, PAs, nurse practitioners, home health aides, physical and occupational therapists, and other clinicians.Travel to patients in their homes or facilities.Keep each patient's physician updated on care needs and progress.Supervise LPNs, home health aides, and personal home care aides as needed.Teach patients and families the home care procedures and safety strategies that keep patients safe at home.Maintain full compliance with HIPAA and all state and federal regulations.Document clinical notes clearly and concisely.
