About the Role
Penn Medicine at Home is offering two tracks for RN employment: Clinical Nurse - NCL or Clinical Nurse II, depending on degree level and years of experience. The Clinical Nurse 2 or Clinical Nurse-NCL is a full partner on the healthcare team, practicing within the organizational professional practice model. This role involves independently providing patient care through the nursing process, with accountability for assigned patients ranging from infancy to the elderly. The nurse demonstrates competencies in quality improvement, evidence-based practice, leadership, patient-centered care, professionalism, safety, technology, and teamwork.
The Clinical Nurse 2 or Clinical Nurse-NCL collaborates with the interprofessional healthcare team to coordinate care across the continuum, focusing on expanding knowledge and skills to provide individualized care. The role includes assuming leadership responsibilities, seeking feedback, and aligning nursing practice with the organizational vision, mission, and goals while complying with regulatory and accrediting standards.
Responsibilities
- Continuous Quality Improvement (CQI): Utilize data and QI methods to identify problems and opportunities to deliver safe, timely, efficient, effective, and equitable care.
- Evidence-Based Practice and Research: Integrate best current evidence with clinical expertise and patient/family preferences for optimal healthcare delivery and system effectiveness.
- Leadership: Collaborate and apply innovative, systems thinking to engage in evidence-based problem-solving and decision-making to promote effective changes within a complex care delivery system.
- Person and Family-Centered Care: Recognize the patient (or designee) as the source of control and a full partner in providing compassionate, coordinated care based on respect for preferences, values, and needs.
- Professionalism: Demonstrate commitment to the nursing profession through lifelong learning, adherence to ANA’s Code of Ethics, participation in professional organizations, and community outreach.
- Safety: Minimize risk of harm to patients, families, providers, and self through system effectiveness and individual performance.
- Technology/Informatics: Utilize appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision-making across the continuum.
- Teamwork: Engage in cooperation, coordination, and collaboration within inter- and intra-professional teams, including virtual teams, to achieve safe, quality patient outcomes.
Requirements
For Clinical Nurse-NCL track:
- Diploma or Associate Degree in Nursing (ASN) from an accredited school of nursing
- 15+ months of relevant professional nursing experience
- Pennsylvania (or New Jersey) RN licensure
- BLS/CPR certification as a healthcare provider (American Heart Association)
- Access to a car, valid driver’s license, and car insurance
For Clinical Nurse 2 track:
- Bachelor of Science in Nursing (BSN) from an accredited school of nursing
- 15+ months of relevant professional nursing experience
- Pennsylvania (or New Jersey) RN licensure
- BLS/CPR certification as a healthcare provider (American Heart Association)
- Access to a car, valid driver’s license, and car insurance
