Home Health RN (Registered Nurse) - Irwin, PA - Weekdays & Full-Time

About the role

Anova Home Healthcare Services is seeking a compassionate and driven Registered Nurse to provide high-quality care to patients in their homes. The role involves providing comprehensive care, utilizing AI systems, coordinating with interdisciplinary teams, and maintaining compliance with regulations.

Responsibilities

• Provide admission, care management, and follow-up visits using the nursing process.
• Utilize AI system for minimal documentation and maximize patient care.
• Build and update the plan of care with the interdisciplinary team.
• Deliver hands-on nursing care, including physical assessments, medication management, wound care, IV therapy, tube feeding, durable medical equipment setup, and patient education.
• Coordinate care with healthcare providers and supervise support staff.
• Document clinical notes clearly and concisely.

Requirements

• Active RN license in Pennsylvania
• CPR certification
• At least 1 year of nursing experience
• Strong organizational and communication skills
• Reliable transportation and valid driver’s license
• Minimal on-call rotation including weekends, evenings, and holidays
• Prior home health experience preferred

Qualifications

• High school diploma or equivalent
• Bachelor’s degree in Nursing preferred

Skills

• Excellent communication and interpersonal skills
• Ability to manage multiple tasks simultaneously
• Proficiency with electronic health records (EHR)
• Knowledge of home health care protocols and standards
