Seeking a Clinical Manager in home health care to lead and support clinical staff, ensuring high-quality, compassionate care in patients' homes.
Responsibilities
- Guiding clinicians
- Supporting clinical decision-making
- Managing documentation accuracy
- Ensuring compliance with Medicare and regulatory standards
- Conducting quality assurance reviews
- Participating in quality improvement projects
- Monitoring outcomes
- Developing performance improvement plans
- Providing staff education on compliance and documentation standards
- Assisting with patient care on limited occasions
- Collaborating with field clinicians to ensure timely documentation and care plan alignment
Qualifications
- RN license
- Minimum 2 years’ home health experience
- CPR certification
- Valid driver’s license
- Strong knowledge of OASIS, CMS PDGM, and home health standards (certifications preferred)
