Home Health Registered Nurse (Part-Time/PRN)

Key Responsibilities

• Developing and implementing individualized care plans with measurable goals based on assessments and physician input.
• Educating patients and families according to their specific needs.
• Training primary caregivers to optimize patient functioning.
• Coinciding ongoing care through documentation and communication with physicians and caregivers.
• Providing direct care using skills such as patient education, wound management, infusions, catheter care, and dressing changes.

Qualifications And Skills Required

• Active RN license in the applicable state.
• Valid CPR or BLS certification.
• Valid driver’s license.

Preferred Qualifications

• Medicare Skilled Nursing experience.
• Basic understanding of Oasis documentation.
• At least 1 year of RN experience in healthcare settings.
