Responsibilities

  • Manages the schedule for all patients and edits schedules for employees calling in sick, ensuring timely reassignment of patients.
  • Initiates infection control forms and uploads them as “Employee Infection Report” in HR Console.
  • Receives second calls from field staff during times clinical supervisors are unavailable and refers clinical questions to Manager or Director as necessary.
  • Maintains the patient hospitalization log, enters coordination notes in electronic medical records, and updates daily for morning huddle.
  • Provides appropriate transfer summary and medication list to the receiving inpatient facility.
  • Completes requested schedules according to agency scheduling guidelines, including discharge visits, OASIS collection, recert visits, and rescheduled visits.
  • Ensures supervisory visits are scheduled according to agency protocol and regulatory mandates.
  • Adjusts service codes related to discharge visits, supervisory visits, and buddy codes considering the entire episode of care.
  • Runs all scheduling reports, including Agent Summary Report and Missed Visit Done on Paper Report.
  • If the PSC is an LPN, receives lab reports, assesses for normality, faxes copies to doctors and pharmacies, informs appropriate staff, and routes to the Medical Records Department.
  • Updates employee schedules daily, considering productivity and geographic issues, and discusses assignments with supervisor/manager for approval.
  • Serves on at least two agency-wide committees per year as requested.
  • Performs other duties as assigned.

Requirements

  • Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
  • Knowledge of electronic medical record.
  • Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.
  • Ability to complete AdventHealth Home Care Orientation and Training, as necessary.
  • High School Grad or Equiv
  • 1+ related experience in medical office.
  • Home Health Agency office experience (Preferred)
