Home Health Occupational Therapist - Full Time

Position Overview Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with interdisciplinary teams and providing updates to optimize patient outcomes. Key Responsibilities Perform initial and ongoing assessments of mobility, coordination, strength, and function.Administer, evaluate, and interpret tests to set functional goals and outcomes.Provide written evaluations and plans of care, documenting progress and adjusting goals as needed.Deliver occupational therapy services in accordance with physician orders.Maintain documentation, modify treatment plans, and plan discharges, including patient education on home exercises.Coordinate services with healthcare team members, physicians, and caregivers.Complete assessments such as OASIS where permitted by regulation.Supervise other personnel (OTA, HHA) as applicable. Qualifications Degree in Occupational Therapy from an accredited program.Current, unrestricted state license as an OT.Valid CPR certification.
