Home Health Occupational Therapist - Full Time

About the role

This full-time Occupational Therapist role involves providing personalized, in-home care to support patients in living independently and safely in their residences. The role offers schedule flexibility with daytime visits from Monday to Friday, covering territories such as Independence, Arcadia, and surrounding cities. Responsibilities

• Conduct initial and ongoing assessments of mobility, coordination, strength, and functional status.
• Administer, evaluate, and interpret tests to set and monitor goals and outcomes.
• Develop and document a comprehensive plan of care, including goals, progress, and discharge plans.
• Deliver occupational therapy services per physician orders, ensuring accurate documentation and compliance.
• Coordinate with healthcare teams, educate patients and caregivers on home exercise plans, and participate in discharge planning.
• Supervise personnel such as OT Assistants and Home Health Aides, and complete OASIS assessments where permitted.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license.
• Valid CPR certification.
• Valid driver’s license.
• Preferred experience: Home health therapy background and OASIS documentation experience.
