Home Health Occupational Therapist - Full Time

About the role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician‑directed home health plans. Collaborate with interdisciplinary team members to optimize patient well‑being through comprehensive evaluations and tailored treatment plans.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength, and function.
• Administer, evaluate, and interpret tests to set functional goals and outcomes.
• Provide written evaluations and care plans outlining therapy needs, goals, and progress.
• Deliver occupational therapy services in accordance with physician orders.
• Maintain plans of care with timely documentation, modify goals as needed, and plan discharges.
• Coordinate discharge planning, including patient education on home exercises.
• Collaborate with healthcare team members, physicians, and caregivers.
• Document effectively to ensure compliance and minimize denials.
• Communicate with staff, families, and serve as a clinical resource.
• Supervise other personnel such as OT Assistants and Home Health Aides, as applicable.
• Complete OASIS assessments where permitted by regulation.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license.
• Valid CPR certification.
• Valid driver’s license.
