Home Health Occupational Therapist - Full Time

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed plans in a home health setting. Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength, and functional levels.
• Administer and interpret tests to establish goals and measure outcomes.
• Develop and document evaluation reports and care plans, adjusting as needed.
• Deliver therapy services in accordance with physician orders.
• Maintain accurate documentation for compliance and billing, ensuring minimal denials.
• Coordinate discharge planning and educate patients and caregivers on home exercise programs.
• Communicate effectively with healthcare team members, patients, and families.
• Supervise other personnel (e.g., OTA, HHA) as appropriate.
• Complete OASIS assessments where permitted by regulation.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Unrestricted state license as an Occupational Therapist.
• Valid CPR certification.
• Valid driver’s license and auto insurance.
