Home Health Occupational Therapist - Full Time

About the role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborate with interdisciplinary teams to optimize patient well-being through evaluations, treatment planning, and documentation.

Responsibilities

• Conduct assessments
• Set functional goals
• Provide therapy services
• Maintain plans of care
• Participate in discharge planning
• Supervise relevant personnel
• Complete assessments like OASIS where permitted
• Develop and modify treatment plans
• Document progress
• Deliver therapy services per physician orders and document effectively
• Coordinate with healthcare team and educate patients and caregivers
• Supervise other personnel as applicable

Qualifications

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license as an OT
• Valid CPR certification
