Home Health Occupational Therapist - Full Time

About the role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within home health care. The role involves collaborating with an interdisciplinary team to optimize patient well-being through assessments of mobility, coordination, and strength, as well as developing and updating personalized plans of care.

Responsibilities

• Assess, teach, and deliver therapy interventions based on physician directives within home health care
• Collaborate with an interdisciplinary team to optimize patient well-being through assessments of mobility, coordination, and strength
• Develop and update personalized plans of care
• Conduct initial and ongoing patient assessments
• Interpret tests and measurements to set functional goals
• Create and update treatment plans and documentation
• Coordinate with healthcare team and provide discharge education
• Maintain effective communication with staff, families, and other providers
• Supervise relevant personnel (e.g., OTAs, HHAs) and complete assessments as allowed
• Document patient progress, modify treatment as needed, and participate in discharge planning
• Educate patients and caregivers on home exercises
• Perform OASIS assessments where permitted
• Ensure compliance with policies

Qualifications And Skills

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license
• CPR certification
• Valid driver’s license
• Proof of auto insurance
• Experience in home health therapy (preferred)
• Experience with OASIS documentation (preferred)
