Home Health Occupational Therapist - Full Time

About the role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within the home health setting. Collaborate with interdisciplinary teams, providing updates to optimize patient outcomes.

Responsibilities

• Conduct initial and ongoing assessments of mobility, coordination, strength, and functional status.
• Administer, evaluate, and interpret tests to set functional goals and measure progress.
• Prepare written evaluations and plans of care outlining therapy needs, goals, and expected outcomes.
• Deliver occupational therapy services per physician orders and modify treatment plans as necessary.
• Maintain accurate documentation, including progress notes, goal adjustments, and discharge planning.
• Collaborate with healthcare team members, patients, and caregivers for coordinated care and education.
• Supervise other personnel such as OT assistants and home health aides where permitted.
• Complete assessments like OASIS where regulation permits.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Unrestricted state license as an Occupational Therapist.
• CPR certification, valid driver’s license, and proof of auto insurance.
• Experience in home health therapy and OASIS documentation preferred.
