About the role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within home health settings. Collaborate with interdisciplinary teams, provide updates, and optimize patient well-being.

Responsibilities

• Conducting assessments of mobility, coordination, and strength
• Interpreting tests to set functional goals
• Creating and updating plans of care
• Delivering therapy services
• Documenting progress
• Managing discharge planning through patient education
• Supervising staff as applicable
• Completing OASIS assessments where permitted

Requirements

• Degree in Occupational Therapy
• Current unrestricted state license
• CPR certification
• Valid driver’s license
