Home Health Occupational Therapist - Full Time

About the Role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within home health care. The role involves collaborating with an interdisciplinary team, updating on patient progress, and providing patient education for discharge planning.

Responsibilities

• Conducting initial and ongoing assessments of mobility, strength, and functionality.
• Administering tests, interpreting results, and setting functional goals.
• Preparing written evaluations and treatment plans outlining needs, goals, and progress.
• Delivering therapy services per physician orders and documenting effectively to ensure compliance.
• Updating plans of care, modifying goals as needed, and planning discharges.
• Collaborating with healthcare team members, patients, and caregivers for coordinated care.
• Supervising other personnel (e.g., OTA, HHA) and completing assessments within scope of practice.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an OT.
• Valid CPR certification.

Preferred Skills

• Strong organizational, communication, and time management skills.
• Experience with home health therapy and OASIS documentation.
• Ability to build relationships and work independently with minimal supervision.
