Home Health Occupational Therapist - Full Time

About the Role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves performing initial and ongoing assessments, creating treatment plans, documenting progress to achieve functional goals, collaborating with interdisciplinary teams, maintaining accurate documentation, and managing discharge planning by educating patients and caregivers. Supervision of personnel such as OT Assistants and Home Health Aides may be required, along with completing OASIS assessments where permitted.

Responsibilities

• Perform initial and ongoing assessments of patients' needs.
• Create and implement treatment plans to achieve functional goals.
• Document progress and maintain accurate records.
• Collaborate with interdisciplinary teams.
• Manage discharge planning, including educating patients and caregivers.
• Supervise OT Assistants and Home Health Aides as required.
• Complete OASIS assessments where permitted.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an OT.
• Valid CPR and driver’s license.
