Role Overview

The Medical Social Worker provides comprehensive social and emotional assessments for home health patients, develops personalized care plans, and coordinates community resources to support recovery. They supervise the Social Worker Assistant, participate in interdisciplinary team activities, and ensure documentation compliance.

Responsibilities

• Assess patients' social, emotional, and financial needs related to their illness or injury.
• Identify and facilitate access to community resources to address barriers to recovery.
• Communicate effectively with patients, families, and healthcare team members.
• Monitor ongoing patient and family needs, adjusting care plans accordingly.
• Participate in care coordination and act as a resource for team members.
• Evaluate social work staff performance and ensure compliance with standards.
• Maintain accurate documentation of patient interactions and care activities.

Qualifications & Skills

• Minimum of one year healthcare social work experience, preferably in home health or hospice.
• State licensing as required, valid driver's license, and reliable transportation.
• Current CPR certification.
• Experience with discharge planning and community resource management.
• Knowledge of medication administration and social service resources.
• Strong organizational, planning, and stress management skills.
• Flexibility for local travel and adaptable work hours.
• Fluent in English, with current licensure and ongoing education compliance.
• This patient-facing role requires TB screening and adherence to driver safety policies.
