Home Health Medical Social Worker - PRN

About the role

Provide personalized social and emotional assessments and develop care plans for home health patients. Evaluate patients' social, financial, and community resources, and coordinate support to facilitate recovery. Communicate with patients, families, and healthcare team members, and supervise the Social Worker Assistant to ensure compliance with standards and documentation requirements.

Responsibilities

• Conduct social and emotional assessments for home health patients.
• Develop and implement individualized care plans.
• Evaluate patients' access to social, financial, and community resources.
• Coordinate support services to aid patient recovery.
• Communicate with patients, families, and healthcare team members.
• Supervise the Social Worker Assistant to ensure compliance with standards and documentation requirements.
• Maintain flexibility and participate in ongoing education.

Requirements

• At least one year of healthcare social work experience.
• Relevant licensure for social work practice.
• Valid driver’s license and reliable transportation.
• Fluency in English.

Pay

$65,000 to $88,600 annually.
