About the Role

The Medical Social Worker collaborates with an interdisciplinary care team to evaluate and develop personalized care plans addressing patients' emotional and social needs in a home health setting.

Responsibilities

• Assess patients' social and emotional states, relationships, and resource needs
• Facilitate access to community resources
• Communicate with patients, families, and team members
• Participate in care coordination
• Supervise Social Worker Assistants
• Document patient progress and care activities

Requirements

• Minimum one year of healthcare social work experience
• Valid social work licensure
• Reliable transportation
• CPR certification
• Knowledge of discharge planning and community resources
• Must pass TB screening
• Maintain valid driver’s insurance
• Fluency in English
• Ongoing licensure and continuing education compliance

Pay

$53,700 to $72,600 annually
