Position Overview
The Medical Social Worker collaborates with the interdisciplinary care team to assess and address the emotional, social, and financial needs of home health patients. They develop personalized care plans, coordinate community resources, and provide ongoing support to facilitate recovery and emotional well-being.
Responsibilities
- Evaluate patients' social and emotional health
- Coordinate community resources
- Communicate with patients and families
- Participate in care planning
- Supervise the Social Worker Assistant
- Maintain thorough documentation in compliance with policies and regulations
Requirements
- At least one year of healthcare social work experience
- Valid licensure
- Reliable transportation
- Current CPR certification
- Proficiency in English
- Must pass TB screening
- Must carry valid driver's insurance
Qualifications
- Experience with discharge planning is preferred
- Medication knowledge is preferred
