Home Health Medical Social Worker-PRN
Reporting to the Clinical Manager, you will participate in the interdisciplinary care provided to home health patients, evaluating and developing personalized care plans to meet patients' emotional and social needs. You will also provide direction and supervision to the Social Worker Assistant when involved in the patient's plan of care.
Responsibilities
- Assess the patient's social and emotional state as it relates to their illness or injury, needs for care, and their response to treatment.
- Evaluate relationships between the patient's medical and nursing needs in the home setting, financial resources, and available community resources.
- Take appropriate action to obtain available community resources to help resolve issues that may impede the patient's recovery.
- Communicate with patients, their family members, and associates within the organization.
- Provide ongoing assessment of patient and family needs and responses to understanding the treatment process and coping with social and emotional challenges.
- Participate as a member of the interdisciplinary care team in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
- Evaluate the performance of the Social Work Assistant (BSW) to ensure all medical social services are provided in compliance with company, government, and professional standards.
- Maintain and submit documentation as required, including case conferences, patient/physician/community contacts, visit reports, progress notes, and collaboration with other healthcare disciplines.
Requirements
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
- Social Worker licensure in the state of practice, if required by state law or regulation.
- Valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Experience with discharge planning and obtaining community resources (e.g., housing, shelter, funeral/memorial services, legal, information and referral, state/federal financial and medication programs, and eligibility).
- Knowledge of medications and their correct administration.
- Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
- Flexibility in work hours and local travel.
- Maintain current licensure, certifications, and meet mandatory continuing education requirements.
- Must read, write, and speak fluent English.
Additional Information
- This role is patient-facing and requires Tuberculosis (TB) screening.
- This role requires a valid state driver's license and maintenance of personal vehicle liability insurance. Minimum coverage: $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 for property damage (or state minimum requirements, whichever is higher).
Pay
The pay range for this full-time (40 hours per week) position is $53,000 - $73,000 per year. The range may vary based on geographic location, and individual pay will depend on demonstrated skills, knowledge, experience, education, and certifications.
