Home Health Medical Social Worker - PRN

About the Role

The Medical Social Worker collaborates within an interdisciplinary care team to evaluate and address patients' emotional and social needs in a home health setting. They develop personalized care plans, coordinate community resources, and support patient recovery by assessing social, emotional, and financial factors. The role involves supervising the Social Worker Assistant, maintaining thorough documentation, and ensuring compliance with policies and regulations.

Responsibilities

• Assess patients' social, emotional, and relational well-being related to their health condition.
• Identify and facilitate access to community resources to support recovery.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in care coordination and interdisciplinary team activities.
• Supervise and evaluate the Social Worker Assistant's performance.
• Maintain accurate documentation, including visit reports and case notes.

Requirements

• Minimum of one year of healthcare social work experience, including home health or hospice.
• Valid licensure as required by state law.
• Current CPR certification, valid driver’s license, and reliable transportation.
• Knowledge of discharge planning, community resources, and medication administration.
• Strong organizational, prioritization, and stress management skills.
• Fluent in English, with flexibility for local travel and work hours.
• Commitment to ongoing education and licensure maintenance.
