Responsibilities

• Lead and support the clinical home health team
• Oversee patient care delivery and ensure compliance with regulatory and accreditation standards
• Coordinate admissions, case conferencing, and care planning to optimize patient outcomes
• Mentor and evaluate field staff, monitor quality metrics, and collaborate with physicians and families
