Home Health Clinical Manager RN
Job Description
Oversee and support clinical operations for adult home health patients, ensuring quality care and regulatory compliance.
Conduct and review comprehensive patient assessments, including Admissions, Start of Care (SOC), Recertifications, Resumption of Care (ROC), Routine Visits, Transfers, and Significant Change in Condition evaluations.
Provide case management oversight and coordinate services with patients, families, physicians, and interdisciplinary care teams.
Ensure accurate and timely completion of clinical documentation, including OASIS assessments and care plan updates.
Monitor and support skilled nursing care for a wide range of patient needs.
Pain assessment and management using verbal and nonverbal pain scales.
Oxygen administration and oxygen safety education.
Post-surgical care, including post-amputation and post-total joint replacement recovery.
Ensure adherence to infection control protocols and clinical best practices.
Provide clinical guidance, problem-solving support, and mentorship to field staff as needed.
Position Requirements
- Active Registered Nurse (RN) license in good standing
- Required experience in Adult Home Health
- Strong experience with admission assessments, case management, and OASIS documentation
- Demonstrated competency in complex skilled nursing procedures and patient care management
- Excellent clinical judgment, organizational skills, and attention to detail
- Ability to work independently and manage multiple priorities in a fast-paced environment
- Travel experience required
- Valid driver’s license and reliable transportation
