About the role
Lakeview Village Home Health provides Medicare-certified, short-term, goal-oriented medical care and support in the comfort of the client’s own home. Services are available to residents of Lakeview Village, as well as to seniors in Johnson, Wyandotte, and Douglas counties.
Responsibilities
- Provide comprehensive nursing services to individuals and their families in a community setting, with emphases on the promotion and maintenance of health, early detection, and restoration measures.
- Lead Home Health Aides and Staff Nurses to ensure proper and effective implementation of care.
- Demonstrate a full understanding and effective application of ICD-10 coding for primary and secondary diagnoses.
- Demonstrate a full understanding and effective application of OASIS assessments. Perform OASIS admissions, transfers, re-certifications, resumptions of care, and discharges.
- Thoroughly and completely record all symptoms, progress, and treatments in the appropriate charting system to ensure the continuity of care, maintenance of accurate legal documents, and caring, professional services.
Requirements
- Graduate of an accredited school of nursing.
- Possess a current, unrestricted RN state nursing license in the state of Kansas.
- Current CPR certification.
- Case Manager certification (preferred).
- Understand and effectively apply state, federal, and medical regulations as they apply to home health.
- Prior home health and/or case management experience (preferred).
- Reliable transportation and a valid state driver’s license.
- Able to work in a team setting with residents, relatives, staff, and others.
- Able to meet the physical demands of the job: Exerts up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects; must have close visual acuity to perform an activity at distances close to the eyes such as: preparing and analyzing data; viewing a computer terminal; extensive reading.
