About the Role

This role operates administratively in a Home and Community-Based Services (HCBS) Setting, defined as “medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs.” HCBS settings provide individuals with the opportunity to live and receive services in a way that honors their abilities. As an HCBS Programmer, you’ll join the mission of empowering people to reach individual achievement across the spectrum of life.

Responsibilities

  • Team Leadership & Culture Development: Build and maintain a strong team culture through active presence, monthly team meetings, staff coaching, on-call participation, and providing coverage as needed, including short-notice staffing gaps.
  • Resource & Operational Oversight: Manage and monitor key resources including vehicle usage, individual budgets, payee-related financial processes, medication check-ins, and appointment scheduling to ensure services run smoothly and efficiently.
  • Person-Centered Service Coordination: Ensure services are delivered in a way that respects individual needs and goals by collaborating with CBCM and IDT, developing individualized plans, completing assessments, and maintaining accurate service documentation and authorizations.
  • Communication & Community Collaboration: Serve as the primary liaison for individuals served, families, guardians, and case managers; communicate service changes promptly, document interactions, and support community partnerships and service promotion efforts.
  • Compliance, Documentation & Advocacy: Maintain compliance with all applicable laws, policies, and regulations; ensure accurate incident reporting and service documentation; and advocate for individuals to promote independence, dignity, and self-determination.

  • Confidence and self-awareness to balance advocacy and leadership.
  • At least 18 years of age.
  • Valid driver’s license and reliable transportation with proof of $100,000/$300,000/$50,000 liability automobile insurance.
  • High School Diploma or GED equivalent.
  • Ability to work in the U.S. without sponsorship.

Physical Demands

  • May require lifting up to 35 pounds; assistance or equipment required for heavier loads.
  • Prolonged sitting and computer work for up to eight hours.
  • Frequent bending, stooping, and stretching.
  • Prolonged standing for up to eight hours.
  • Must wear hearing and eye protection in required areas.
  • Ability to see objects/persons at a distance and read close material (e.g., typed or handwritten).
  • Ability to hear conversations in quiet and noisy environments and determine sound direction.
  • Ability to communicate through speech.
  • Must drive and transport for service provision as needed.
  • Must remain awake during working hours, if applicable.

Pay

Base pay is $22.63/hour, with potential for higher starting wages based on education and experience.

Schedule

This full-time position operates Monday through Friday, 8:00 a.m. to 4:30 p.m. The role includes participation in an on-call rotation and may require flexibility to provide additional coverage as needed in site homes.
