Home-Based Physical Therapist

Role: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions, adhering to professional standards and ethical practices. Conducts evaluations and administers treatments as prescribed, working with patients across all age groups.Delivers therapies such as muscle strengthening, gait training, ADL training, and manages orthotic and prosthetic devices.Performs initial and periodic evaluations, documents progress, and develops tailored treatment plans with clear goals.Ensures accurate, timely clinical documentation compliant with department and payer guidelines.Instructs patients and caregivers on activities between treatments using clear written and verbal instructions.Communicates regularly with healthcare team members, providing updates and coordinating care adjustments.Manages appointment scheduling, transportation logistics, and maintains portable therapy equipment.Maintains department policies, participates in quality improvement, and manages productivity expectations.Monitors patient authorizations and insurance coverage, addressing scheduling or insurance concerns.Performs additional duties as assigned. Qualifications High School Diploma or GED required.Doctor of Physical Therapy (DPT) degree required, with completion after 2015.Licensed Physical Therapist and valid Texas Driver's License required.Auto liability insurance and BLS certification (to be obtained within 90 days). Skills & Experience: No prior experience needed; strong communication, organization, and transportation reliability are essential.
