Role Overview

The OP Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, adhering to professional standards and ethical practices.

Responsibilities

• Conduct evaluations and treatments for patients across all ages
• Perform initial and ongoing assessments
• Develop and adjust individualized treatment plans with clear goals
• Ensure accurate documentation of patient progress
• Provide therapy such as muscle strengthening, gait training, ADL training, and orthotic/prosthetic management
• Educate patients and caregivers on activity routines
• Communicate with healthcare providers
• Coordinate scheduling efficiently
• Maintain therapy equipment
• Adhere to departmental policies
• Monitor insurance coverage
• Manage productivity expectations
• Ensure safety, quality improvement, and program development
• Maintain reliable transportation for home visits

Qualifications & Skills

• High school diploma or GED
• Valid Texas Driver's License
• Certification in Basic Life Support (BLS)
• Doctor of Physical Therapy (DPT) required for post-2015 graduates
• Licensure as a Physical Therapist
• Auto liability insurance
